ミオグロビン尿症による急性腎不全の4例

FOUR CASES OF ACUTE RENAL FAILURE CAUSED BY MYOGLOBINURIA

　ミオグロビン尿症による急性腎不全を4例経験した。原因は過激な運動，抗精神薬，外傷，抗不整脈剤であった。全例に，ハイパフォーマソス膜を使用した血液透析，血液濾過，吸着濾過を行い，3例は腎機能が回復し，1例は汎血管内凝固症候群で死亡した。血中ミオグロビン値は腎機能を反映する指標として有用であり，早期に血液透析を開始することが治療上重要である。しかし，外傷性は多臓器不全を起こすことが多く，本例でも救命できなかった。

We reported four cases of acute renal failure caused by myoglobinuria, which was induced by hard exercise, excessive major tranquilizer administration, trauma and drug induced myositis respectively. Using high performance membrane, HDF,and absorptive serum perfusion were performed as an intial treatment in all cases. In 3 cases renal function impoved but 1 case with trauma died because of DIC complication. It is clear that early treatment is most important and value of myoglobin level in blood seems to reflect the recovery of the renal function more procisely than serum creatinine.
