停留睾丸捻転により発見されたセミノーマの1例

SEMINOMA DISCOVERED BY TORSION OF THE UNDESCENDED TESTIS: A CASE REPORT

　24歳，男性。主訴は左鼠径部腫脹と疼痛。1987年5月14日突然左鼠径部痛出現，翌日当科受診。触診にて左鼠径部に超鶏卵大の圧痛を伴う腫瘤を触知。超音波検査にて充実性の腫瘤を認めた。鼠径部停留睾丸腫瘍の捻転の疑いにて，5月25日手術を施行。睾丸固有鞘膜内に反時計回りに270度回転した睾丸腫瘍を認めた。組織型はセミノーマで，T₁N₀M₀，stage Ⅰと診断した。停留睾丸捻転症の睾丸腫瘍合併例は本邦文献上6例目になる。

A 24-year-old man visited our department of Hiroshima City Hospital complaining of It. inguinal pain and swelling. Over-hen's egg sized tumor was palpable and US proved heterogeneous solid mass with a small amount of fluid. Lt. orchiectomy was done with the findings of testicular tumor which had 270° counter-clockwise intravaginal rotation. Histopathological findings proved pure seminoma. Our case was thought to be the sixth case in the Japanese literature.
