睾丸垂・副睾丸垂同時同側捻転症

SIMULTANEOUS IPSILATERAL TORSION OF THE APPENDICES TESTIS AND EPIDIDYMIS

　48歳，男性。左陰嚢部痛および左鼠径部痛を主訴に当科受診。睾丸シンチグラムで，左睾丸の血流低下が示唆されたため，左睾丸捻転症を疑い，緊急手術を施行。睾丸捻転の所見はなく，左睾丸垂および副睾丸垂捻転症と判明し，垂をそれぞれ摘除後，睾丸固定術を施行した。術後8日目の睾丸シンチグラムでは，左睾丸の血流は対側と同程度まで回復していた。睾丸垂と副睾丸垂の同時同側捻転例の報告は，自験例が本邦第1例と思われる。

A 48-year-old male was admitted to our department with the chief complaint of pain in the left scrotum and groin. Because testicular scintiscanning suggested vascular hypoperfusion of the left testis, left testicular torsion was suspected. Exploratory operation revealed torsion of the left appendices testis and epididymis. They were extirpated and orchidopexy was performed. Eight days after, testicular scintiscanning showed recovery of vascular perfusion of the left testis. This is the first recorded case of simultaneous ipsilateral torsion of the appendices testis and epididymis in Japan.
