停留睾丸に合併した副睾丸垂捻転症の1例

A CASE OF TORSION OF THE APPENDIX EPIDIDYMIS IN A MALDESCENDED TESTIS

　症例は22歳，男性。生来，左睾丸の欠如に気づいていた。1985年5月19日より，左鼠径部に間歇的な痛みを覚えるようになつた。2日後疼痛が増強し持続性となつたため，左停留睾丸に合併した睾丸回転症を疑い，緊急手術を施行した。その結果副睾丸垂捻転症とわかり，副睾丸垂を切除し，睾丸固定術を施行した。停留睾丸に合併した副睾丸垂捻転症の報告例は，自験例を含めて本邦で2例，世界でも4例であり稀なものであつた。

A 22-year-old man was admitted because of pain in the left inguinal lesion for the preceding two days. On examination a soft tender mass was felt in the left inguinal lesion and the left testis was absent in the scrotum. Torsion of a maldescended testis was diagnosed. At operation the left testis was found almost normal in the inguinal canal with a gangrenous appendage arising from the head of the epididymis. The appendage was excised and orchiopexy was performed.
