54歳男性，生来左停留睾丸を指摘されるも放置，1984年，胃癌の診断にて胃全摘を施行，その約1年後左停留睾丸に一致して，透光性のある手拳大の鼠径部睾丸水瘤を認めるようになつた。水瘤液細胞診，超音波検査，WBCTより，胃癌の左停留睾丸への転移，またそれに伴う睾丸水瘤を疑い，手術を行つた。術後病理組織診では，左副睾丸への胃癌転移と診断され，転移経路としては，リンパ管逆行性転移が考えられた。
A 54 year old male was admitted with the complaint of the enlargement of the left undescended testis who had a history of operations for gastric cancer a year previously. Physical examination revealed a firm non-tender unmovable mass with transillumination, about 7 cm in diameter in the left inguinal portion. Ultrasonography and CT showed left undescended testis with hydrocele testis. Percutaneous needle aspiration of the hydrocele fluid was performed for cytology which revealed adenocarcinoma.
