帯状庖疹により排尿障害と勃起障害を生じた症例

BLADDER DYSFUNCTION AND ERECTILE IMPOTENCE WITH HERPES ZOSTER

　24歳，男性。仙髄領域（S3〜S5）右側の帯状疱疹による尿閉症例。膀胱粘膜にも頸部付近の両側に疱疹を認めた。間歇的自己導尿を行い，約1カ月後にはほぼ正常排尿となつた。その後勃起障害を訴えた。経時的な球海綿体筋反射の潜時測定と，パパベリン海綿体内注入に超音波ドプラー法を組み合せた簡便な血流動態検査により検討した。約5カ月後には，軽度の末梢神経障害は残すものの，正常に近い勃起と性交が可能となつた。

A 24-year-old male with herpes zoster involved from the 3rd to 5th sacral segments on the right side and eruption on bilateral side of the bladder neck had urinary retention and also became erectile impotent. One month later, he was possible to empty his bladder after repeated intermittent self cathe-terization. We estimated the sequential BCR-EMG latency time and the simple penile hemodynamics using doppler ultrasonic flowmeter combined with the intracavernous injection of papaverine. Although, his sacral peripheral nervous system was revealed slightly impediment, he got spontaneously well sexual life after 5 months treatment.
