ホルモン抵抗性前立腺癌に対する抗癌剤動注療法の検討

EFFECTIVE TREATMENT OF INTRA-ARTERIAL ANTICANCER DRUGS FOR HORMONALLY-UNRESPONSIVE CARCINOMA OF THE PROSTATE

緒言

　悪性腫瘍に対する抗癌剤の局所動脈内注入療法の目的は，全身投与では不可能な高濃度かつ大量の薬剤を腫瘍の支配血管に注入することにより，少ない副作用で治療効果を高める点にある。またこの際全身的にも薬剤は分布するので，全身投与と同じ効果もある程度期待できる。

　われわれは，従来一般に行なわれてきたホルモン療法に抵抗を示す前立腺癌に対し，上臀または下臀動脈よりカテーテルを留置して，長期間抗癌剤の注入療法を施行した。好結果をえたので，手技，治療内容，副作用および問題点などにつき報告する。

Nine patients with hormonally-unresponsive carcinoma of the prostate were treated with intra-arterial anticancer drugs. Two cases were stage C and seven cases were stage D. Catheter was inserted into the superior or inferior gluteal artery and drugs were injected mainly into the inferior vesical artery. We used diethylstilbestrol diphosphate, Peplomycin and Cis-platin. According to the evaluation of therapy of NPCP, objective partial response was documented in 3 patients and 6 were objectively stable. This response lasted from 2 to 36 months with an average of 19 months.
