原発性前立腺移行上皮癌の1例

A Case of Primary Transitional Cell Carcinoma of the Prostate

　72歳，男性。主訴は肉眼的血尿。4年前より前立腺腺癌（stage A2）の診断でLH-RH製剤の投与を受けていた。血中PSA値は正常範囲内であったが，骨盤CTで前立腺部に腫瘍が認められたため，生検を行ったところ，移行上皮癌との診断で当科に紹介された。膀胱無作為生検で癌はなく，原発性前立腺移行上皮癌と診断し，化学療法および放射線療法を行った。原発性前立腺移行上皮癌は稀であり，自験例は本邦29例目に相当する。

A 72-year-old patient with prostatic adenocarcinoma had been well controlled by LH-RH agonist for 4 years. When he complained of gross hematuria in August 1997, excre-tory urography (IVP) revealed no abnormalities. Pelvic CT scan revealed tumorous lesion at the prostate. Serum prostatic specific antigen (PSA), however,was non-con-tributory. Histological diagnosis of the specimen obtained at the time of transrectal prostate biopsy was transitional cell carcinoma.
