尿道内圧曲線に対する尿道周囲臓器および腹圧の影響について

A STUDY ABOUT INFLUENCES OF LAPAROTOMY AND ORGANS SURROUNDING URETHRA ON URETHRAL PRESSURE PROFILE

緒言

　尿道内圧測定（UPP）は今やurodynamic検査上欠かせないものになりつつあるが，UPP自体は単に尿道固有の圧記録というより，周囲臓器を含めてのtotalな尿道内圧記録であるにもかかわらず，UPP波形を構成するcomponentsについては，何となく膀胱頸部の立ち上がりとか，外括約筋部がUP maxを表わしているとか言われているだけで，腹圧がどのように影響しているか，直腸の収縮とどのような関係にあるのかというような，尿道の周囲臓器とUPPとの関連についてはあまり検討されていない。

　われわれは犬膀胱尿道を直視下に観察して種々の実験を行なつているが^1），その手術操作は，開腹して恥骨結合を除去し，直腸と尿道をも剥離して，尿道固有の動きを見るものである。そこで，今回は開腹操作，直腸の動きなど尿道周囲の要素がUPP曲線にどのような影響を与えるかを検討することによつて，Urodynamics検査上UPPの占める役割に関して考えてみた。

How the operative procedure such as laparotomy and resection of pubic bone influences the uretheral pressure profile (UPP) was studied on mongrel dogs. The pressure of the external sphincter remarkably dropped after laparotomy. While the pressure of the internal sphincter decrcsed by resection of pubic bone. The urethral pressure in total urethral length descended after administration of suxa-methonium. These result demonstrated that the UPP shows the total urethral pressure including the pressure of surrounding organs, pressure caused by rectal contraction and abdominal pressure.
