性染色体構成XX型の真性半陰陽の1例

A CASE OF TRUE HERMAPHRODITE WITH XX SEX CHROMOSME CONSTITUTION

Ⅰ．緒言

　性に関する研究は，遺伝学の進歩とともに発展し，1949年Barr等^1）のSex chromatinに関する研究で一躍進歩をとげ，現在では性の決定は，性染色体の組合わせにより行なわれるとされている。しかしながら，胎生のごく初期においては性腺原基は性染色体の組合わせが男であれ女であれ将来睾丸にも卵巣にも発育できる両要素を有しており，正常な個体では性染色体に一致した要素のみが分化発育し反対の要素は退化萎縮するわけであるが，何等かの原囚で両要素が発育してくることが当然起こりうるわけである。このような先天的な異常を真性半陰陽と呼んでいる。

　本症は，極めてまれな疾患であり，本邦においては1923年前田^2）の報告以来現在までわれわれが蒐集しえた症例は自験例をふくめてわずか64例をかぞえるに過ぎない。

Case is a 25 years old, socialy male, with the chief complaint of abnormality of the external geni-talia and gynecomastia. Family and past history was irrelevant. Abnormality of the external genitalia was noticed at birth but nothing was done and the gynecomastia appeared at 12 with the conture shape of the entire body. Urination had to be done by female position and was admitted to our clinic for correction of the abnormal genitalia.

Present status and local findings revealed good development of subcutaneous fat with complete female posture and the breast were those of the matured female.
