The most severe postoperative problems after total glossectomy are speech and swallowing dis-orders. In a 51-year-old man with recurrent meso-pharynx cancer, MRI revealed a mass in the right side of the base of tongue. The mass extended to the right side of the oral floor. Pathological examina-tion of the mass showed recurrence of squamous cell carcinoma. The patient underwent total glos-sectomy and the tongue was reconstructed with a recto-abdominal myocutaneous free flap by means of plastic surgery techniques. The patient's speech and swallowing ability became acceptable after surgery.
