Submental Island Flapが咽頭・皮膚・舌再建に有用であった4例

Four cases of head and neck cancer surgery which used Submental Island Flap for pharynx, skin, and tongue reconstruction

はじめに

　Submental Island Flap：SIF（オトガイ下皮弁）は，顔面動静脈の分岐であるオトガイ下動静脈を栄養血管にもつ頸部有茎筋皮弁である。過去の報告からは，手技が比較的容易であるため術者ごとの経験の偏りは回避でき，皮弁の生着も良好であること，さらに術後の創部の審美面からもメリットが多いことが示唆されている。1990年に提唱されて以来^1），本邦でも徐々に浸透してきたが，まだ十分な症例数が報告されていない。今回われわれは，耳前部皮膚癌再発1症例，舌癌2症例，喉頭全摘術後の下咽頭癌1症例の合計4症例にSIFを用いて再建した。咽頭再建・顔面頸部の皮膚再建・舌再建のいずれにおいても有用であったため報告する。

　The Submental Island Flap（SIF）refers to a cervical caulescent musculocutaneous valve which is given nutrients by the submental artery. Because it is recommended that use of SIF does not involve internal jugular vein invasion and cervical lymph node metastasis of the Level I region, the number of adaptation cases are limited. However, reconstruction by SIF is relatively easy and is superior in functional aspects. We report on 4 cases（including 1 recurrence）which used SIF for reconstruction after head and neck cancer extraction. We rebuilt pharynxes, skin, and tongues using SIF；all cases showed good survival.
