A 54-year-old man with squamous cell carcinoma of the tongue was treated by total glossectomy, including the removal of the hyoid bone and lingual surface of the epiglottic mucosa, but without laryn-gectomy. Oral cavity reconstruction with a free latissimus dorsi myocutaneous flap, laryngeal sus-pension, cricopharyngeal and sternothyroid myotomy was performed to prevent aspiration. Anterior traction of the epiglottis was also perfor-med to prevent laryngeal inlet overclosure due to the weight of the myocutaneous flap sutured to the epiglottis.

Postoperative fluorography confirmed disappear-ance of the vallecula epiglottica, and laryngeal inlet was hidden by the reconstructed base of tongue. Food descended quickly through the post-laryngeal area without aspiration.
