A 58 year old male with hyperosmolar non-ketatic Coma (HNC) following total laryngectomy is reported. A diagnosis of HNC was made based on the following results of laboratory tests : serumlevel of glucose 988 mg/dl, Na 168 mEq/l, K 2.7 mEq/l, plasma osmolarity 116.3 mOsm/kg, and neg-ative keton bodies. He was treated with continuous intravenous drip infusion and insulin. With this therapy, he recovered from the syndrome after 22 days.

Patients after total laryngectomy have tended to develop dehydration, because they can not drink when they want, due to aphonia. Consideration must he given to HNC when managing these patients.
