舌紡錘細胞癌の1例

Spindle Cell Carcinoma of the Tongue : Report of a Case

　はじめに

　頭頸部領域における悪性腫瘍は癌が最も多く，次いで肉腫である。上皮性悪性腫瘍で紡錘細胞を主体とする一見肉腫様の成分を有するいわゆる癌肉腫は，1864年Virchow^1）が癌肉腫として最初に報告した。しかし現在では電子顕微鏡および免疫組織化学的組織分類により紡錘細胞は上皮由来の細胞と考えられ，紡錘細胞癌（spindle cell car-cinoma）という名称が一般的に用いられている。頭頸部領域では喉頭に発生したものはしばしば報告されているが，舌は喉頭に比べ発生頻度は低い。われわれは舌に発生し，組織学的診断が困難であった紡錘細胞癌の1例を経験したので報告する。

A 36-year-old woman presented with spindle cell carcinoma of the tongue. This tumor is rare in the head and neck region. The histogenesis of thistumor is still controversial. It is generally believed that spindle cell carcinoma is a variant of squamous cell carcinoma from immunohistochemical and ultrastructural studies. Immunohistochemical study in the present case showed that the tumor cells revealed both epithelial and nonepithelial character-istics. The presence of desmosomes and transitional forms from squamous to spindle cells strongly sug-gested that spindle cells were epithelial in their origin.
