Two eyes developed pigment dispersion syndrome following cataract surgery with intraocular lens (IOL) implantation. In the first case, the syndrome developed by rubbing the iris by rotating three-piece IOL fixated out of the bag. The intraocular pressure normalized after removal of the IOL and secondary implantation of a 13.5mm one-piece IOL. The other case showed ruptured posterior capsule, vitreous herniation and ectopic pupil. The syndrome was apparently caused by rubbing the iris by the IOL fixated out of the bag. Trabeculotomy led to normalized intraocular pressure. The cases illustrate that inadequately fixated out-of-the-bag IOL may induce pigment dispersion syndrome.
