（展示72）　眼内レンズを挿入する白内障手術後の2眼にpigmentdispersion syndromeが発症した。1例は嚢外に挿入した3ピース眼内レンズが回転して虹彩を擦って発症した。眼内レンズを摘出し，13.5mmの一体式眼内レンズと入れ替えて眼圧が正常化した。他の1例では，後嚢破損青硝子体脱出，瞳孔偏位があり，嚢外に挿入した眼内レンズが虹彩を擦って発症したと推定された。トラベクロトミーで眼圧が正常化した。不適切な眼内レンズの嚢外固定による本症候群は警戒されるべきである。

Two eyes developed pigment dispersion syndrome following cataract surgery with intraocular lens (IOL) implantation. In the first case, the syndrome developed by rubbing the iris by rotating three-piece IOL fixated out of the bag. The intraocular pressure normalized after removal of the IOL and secondary implantation of a 13.5mm one-piece IOL. The other case showed ruptured posterior capsule, vitreous herniation and ectopic pupil. The syndrome was apparently caused by rubbing the iris by the IOL fixated out of the bag. Trabeculotomy led to normalized intraocular pressure. The cases illustrate that inadequately fixated out-of-the-bag IOL may induce pigment dispersion syndrome.
