(P2-2-47) 過去10年間の穿孔性眼外傷の自検例105眼104例を検索した。12歳以下の低年齢層では約半数が他人による受動的な外傷で,65歳以上の高年齢層では転倒が多かった。13歳から64歳までの青壮年層では草刈り機や金属工具などを使った作業中が多かった。これらは防護眼鏡装用で予防できたと思われた。受傷から6か月以上の56眼での視力転帰は,鋸状縁よりも後方の例で悪く,手術までに時間が経過した症例で特に不良であった。穿孔性眼外傷には早期の硝子体手術で対応することが望ましい。
We reviewed the clinical course of penetrating ocular trauma in 105 eyes of 104 patients during the foregoing 10-year period. Passive injury by others was the cause in about 50% of cases aged 12 years or less. Falling down was the major cause in persons aged 65 years or older. Injury occurred in the majority of cases aged 13 to 64 years while working with grass mower or metal-processing instruments. It appeared that injury could have been avoided by wearing protective glasses. In 56 eyes which were treated later than 6 months after injury, visual outcome was poor when perforation involved the posterior ocular segment and when surgery was performed at a later date. We advocate early vitreous surgery for penetrating ocular trauma as the treatment of choice.
