原発性開放隅角緑内障に対する2回目トラベクロトミーの成績

Results of trabeculotomy as a second surgical procedure for primary open angle glaucoma

　原発性開放隅角緑内障で薬物療法により眼圧コントロールが不十分な症例60眼に対してトラベクロトミーを過去9年間に行った。その後23眼が眼圧コントロール不良となり，うち10眼に対して2回目のトラベクロトミーを追加した。

　術後3年で，眼圧コントロールが良好な率は初回トラベクロトミーで59％であり，2回言トラベクロトミーでは30％であった。2回目トラベクロトミーは初回手術より長期予後について劣っているが，術前高眼圧，低年齢などトラベクロトミーのききにくい因子をもつ症例が多く含まれているからと考えられた。

We treated 60 eyes with primary open angle glaucoma by trabeculotomy during the foregoing 9 -year period. We performed additional trabeculotomy in 10 out of the 23 eyes with poor intraocular pressure (IOP) control after the initial surgery.

Three years after initial trabeculotomy, suffi-cient IOP control was achieved in 59%. Three yearsafter second trabeculotomy, it was achieved in 30%.

The second trabeculotomy was inferior to the initial surgery in respect to the longterm progress. It was due to many cases in which trabeculotomy was not effective by nature. It was supposed that those factors were the low age, high IOP before the operation, etc.
