糖尿病硝子体手術後の黄斑症の改善

Improvement of maculopathy after diabetic vitrectomy

　後部硝子体膜による網膜接線方向の牽引力が原因と考えられる両眼性の糖尿病性黄斑症に対して，片眼のみに硝子体手術を行い，術後の黄斑症改善の過程を非手術眼と比較した。1例は片眼に裂孔原性網膜剥離，他の例は片眼に硝子体出血をきたしたために硝子体手術を行った。術中所見では，黄斑部網膜に肥厚した後部硝子体膜が癒着しており，後極部から周辺部に向かって人工的後部硝子体剥離を作成した。手術眼では，術後黄斑浮腫および網膜下滲出物は徐々に減少し矯正視力は改善したが，非手術眼では黄斑症の改善がみられなかった。螢光眼底検査では，手術眼は非手術眼に比較して明らかな螢光漏出の減少をみた。後部硝子体膜による網膜接線方向の牽引が黄斑浮腫の原因と考えられる症例では，硝子体手術により黄斑直上の硝子体を除去することで黄斑症の改善する可能性が示唆された。

We performed unilateral vitrectomy with mem-brane peeling in 2 patients with bilateral diabeticmaculopathy and traction with thickened posteriorhyaloid membrane. One case also manifestedvitreous hemorrhage and the other rhegmatogenousretinal detachment. The operated eyes showedgradual resolution of macular edema and subretinalexudation with improvement in vision. Theseimprovements did not set in the fellow eyes. Fluor-escein angiography showed decrease in diffusemacular leakage in the operated eyes. The findingsillustrate the usefulness of vitrectomy for diabeticmacular edema and traction by thickened posteriorhyaloid membrane.
