角膜混濁を有する症例に対する硝子体手術

Vitreous surgery for eyes with opaque cornea

　角膜混濁を有する10症例10眼に対する硝子体手術成績について検討した。一時的人工角膜を用いて，全層角膜移植と同時手術を行った症例の成功率は低く，2段階以上の視力改善は37.5％に得られたのみであった。一方，同時手術を術前予定していたが，硝子体手術のみを行い得た症例が2例あり，予想していた以上に眼底は透見でき，硝子体手術については大きな支障はなかった。

　また，無硝子体眼に対する全層角膜移植手術の透明治癒率は比較的良好であるので，角膜混濁を有する症例に硝子体手術が必要な場合には，一時的人工角膜を用いた全層角膜移植との同時手術を計画する前に，硝子体手術のみが可能かどうか慎重に検討すべきと思われる。また，可能であれば，視力回復のための角膜移植は網膜疾患が安定してから行えばよいと考えられた。

We reviewed the results of vitreous surgery in 10 eyes with opaque cornea. We performed combined pars plana vitrectomy and penetrating keratoplasty using temporary keratoprosthesis in 8 eyes. Simple vitrectomy was performed in 2 eyes. The treatmentwas successful in 3 eyes, 38%, in the former group and in all in the latter. Because of the poor outcome in the former group, we advocate simple vitrectomy for eyes with opaque cornea so long as the ocular fundus can be seen. All other possibilities should have been considered before deciding for vitrectomy with simultaneous temporary kerato-prosthesis.
