角膜ヘルペスにおける眼圧上昇

Ocular hypertension in herpes simplex keratitis

　1989年までの10年間に，樹枝状角膜炎が観察された症例のうち，21mmHgを越える眼圧上昇例34例を対象に，眼圧上昇の経過と，眼圧上昇時の角膜所見について検討した。眼圧上昇は中等度に留まり，ステロイド剤内服開始から，50％が2週間以内で，眼圧は正常化し，比較的後遺症なく終息した。34例中26例の輪部に隆起を伴う毛様充血，角膜輪部の浮腫，混濁があり，その部の角膜後面沈着物を観察した。この角膜輪部所見は，眼圧上昇例で有意に多く，これらは眼圧上昇に関連することが示唆された。

Through a review of 284 eyes with herpes sim-plex keratitis, we identified 34 eyes with increasedintraocular pressure (TOP). These 34 eyes alsomanifested keratic precipitates and moderate cellu-lar reaction in the anterior chamber. The IOP levelaveraged 30.4 mmHg. Response to systemic cor-ticosteroid was prompt. Normalization of IOP wasattained within 2 weeks in 50% of the eyes. Out ofthe 34 eyes, 26 (77%) manifested various inflamma-tory signs in the vicinity of limbal regions asedema, swelling or infiltration. These limbal lesionsseemed to be causatively related to the increasedIOP.
