眼圧上昇を伴った角膜内皮炎の1例

A case of corneal endotheliitis with elevated intraocular pressure

　症例は37歳の女性で，右眼の視力低下を主訴に島根医科大学附属病院眼科に紹介された。初診時の矯正視力は右0.03，左1.2眼圧は右29mmHg，左12mmHgで，右角膜は中央部の上皮に小水胞，全層に浮腫を認めた。細胞浸潤はなかった。角膜内皮に強いスリガラス状の混濁がみられ，混濁部位に一致して後面沈着物を認めた。高眼圧に対してβ遮断薬の点眼を行い眼圧は下降した。角膜病変はステロイド薬，アシクロビルの局所投与に反応せず，アシクロビルの内服投与で軽快した。最終的にヘルペス性中央部浮腫聖角膜内皮炎と診断した。中央部浮腫型角膜内皮炎は，眼圧上昇を伴いステロイド薬やアシクロビルの局所投与に反応しないことがある。そのような症例にはアシクロビルの内服投与も考慮すべきと思われた。

A 37-year-old woman was referred to us for blurred vision in her right eye. Her visual acuity was0.03 right and 1.2 left. Intraocular presure was 29mmHg right and 12mmHg left. The right eye showed epithelialand stromal edema in the central cornea. Cell infiltration was absent. The corneal endothelium was hazy withkeratic precipitates in the affected area. The intraocular pressure was contolled by topical beta -blocker. Thecorneal lesions failed to improve by topical corticosteroid and acyclovir but responded well to systemic acyclovir.These findings led to the diagnosis of herpetic corneal endotheliitis.
