塩酸ニカルジピン（ペルジピン®）注による術中高血圧の治療

Control of systemic hypertension during surgery with intravenous nicardipine hydrochloride

　白内障などの眼科手術中の異常高血圧を塩酸ニカルジピン（ペルジピン^®）の静注により治療し好結果を得た。

　対象は年齢50〜83歳の白内障患者20名（延べ22例）で，白内障手術中に収縮期血圧が190mmHg以上の高血圧を呈した症例である。投与量は1mgで，投与直前の収縮期血圧203.2±11.1mmHgが投与5分後には150.0±16.4mmHgに下がり，以後安定し投与30分後でも156.2±18.3mmHgであった。1mgの追加投与した症例が2例だけあった。心拍数は投与前80.1±15.7が投与5分後に87.0±14.2と増加したが，投与10分後には80.2±14.1に戻った。塩酸ニカルジピンは重篤な副作用もなく，点滴ルートから静注できるので，白内障などの眼科手術中の高血圧のコントロールに適していると考えられる。

We treated acute rise of blood pressure duringcataract surgery with intravenous injection of 1 mgof nicardipine hydrochloride. This procedure wasapplied to 20 cases in 22 sessions of cataract surgery. The age of patients ranged from 50 to 83years. Nicardipine hydrochloride was administeredwhenever the systolic blood pressure exceeded 190mmHg during surgery. In these 22 sessions, thesystolic blood pressure of 203±11 mmHg droppedto 150±16 mmHg 5 minutes after injection. thedecreased blood pressure remained stable for 30minutes after injection. An additional injection wasrequired in 2 instances only. The initial heart rate,80.1±15.7 per minute, reached a peak 5 minutesafter injection at 87.0±14.2, to decline to 80.2±14.1 five minutes later. This procedure thusappeared to be safe and effective in the management of acute rise of systemic blood pressureduring eye surgery.
