ぶどう膜炎に対する硝子体手術の成績

Pars plana vitrectomy in uveitis

　桐沢型ぶどう膜炎（acute retinal nec—rosis）を含む種々のぶどう膜炎27眼に対して硝子体手術を行った。対象としたのは網膜剥離が15眼，硝子体混濁が7眼，硝子体混濁を伴う黄斑部網膜上膜が2眼，硝子体出血が3眼である。21眼で視力が改善したが，3眼では視力を喪失した。ぶどう膜炎の症例に対する硝子体手術は桐沢型ぶどう膜炎と網膜剥離に対しては積極的に行う必要があるが，網膜剥離を伴わない硝子体混濁や硝子体出血に対しては慎重に適応を検討して行うことが必要だと考えられる。

We performed pars plana vitrectomy in 27 eyes with various types of uveitis including retinal detachment 15 eyes, vitreous opacity 7 eyes, vitre-ous opacity with epiretinai membrane 2 eyes and vitreous hemorrhage 3 eyes. After surgery, the visual acuity improved in 21 eyes and was lost in 3. We advocate early vitrectomy in uveitis with retinal detachment and in acute retinal necrosis. In eyes without retinal detachment, vitrectomy is recommended to relieve vitreous opacity or hemor-rhage after systemic corticosteroid has proved futile.
