サルコイド性ぶどう膜炎患者のEpstein-Barrウイルス抗体価

Antibody titers for Epstein-Barr virus in uveitis patients with sarcoidosis

　京大病院眼科ぶどう膜炎外来に通院中のサルコイド性ぶどう膜炎患者23名（前部ぶどう膜炎患者7名，全ぶどう膜炎患者16名）眼症状のないサルコイドージス患者13名および対照健康人144名について，血清Epstein-Barr （EB）ウイルス抗体価を測定した．抗Early-Antigen抗体の抗体上昇例（抗体価≧20）出現率，および抗体価の幾何平均とも，前部ぶどう膜炎患者群では対照健康人群よりP＜0.001で有意に高値を示した．また有意差は認められなかったが，全ぶどう膜炎患者群と比べても高値を示した．抗Viro-Capsid-Antigen抗体価，抗EB Virus Nuclear Antigen抗体価の幾何平均は前部ぶどう膜炎患者で全ぶどう膜炎患者と比較し高値を示し，先に報告した一般のぶどう膜炎と同じパターンを示した．EBウイルスに対する免疫反応において，サルコイド性ぶどう膜炎患者群における前部ぶどう膜炎患者群と全ぶどう膜炎患者群とは，全く異なった抗体価のパターンを示したことから，一般のブドウ膜炎と同様にサルコイド性ぶどう膜炎においても両者の間には免疫応答に相異があるのではないかと考えた．

We determined the antibody titers for Epstein- Barr-virus in the serum in 39 patients with sarcoidosis. Pan-uveitis was present in 16 cases, anterior uveitis in 7 and no ocular involvement in 13. Patients with anterior uveitis showed higher mean titers for anti-viro-capsid antigen and anti Epstein-Barr-virus nuclear antigen than those with pan uveitis. The observed differences resembled the different antibody patterns between ante-rior.uveitis and pan uveitis in our earlier study.

Patients with anterior uveitis showed significantly higher values in the frequency of sera with elevated titer and in the geometric mean titer for anti-early antigen than control subjects or patients with pan uveitis (p<0.001).

These findings are in accord with our earlier inter-pretation that anatomical and immunological differ-ences do exist between anterior uveitis and pan uveitis even though both belong to the same clinical entity of sarcoid uveitis.
