ベーチェット病の併発白内障に対する手術成績

Surgical outcome of complicated cataract in Behçet's disease

　併発白内障に対して手術を施行し，1年以上の経過を観察したベーチェット病29眼（18例）の視力予後をretrospectiveに検討した。7眼では最終視力が0.5以上となったが，一方，7眼は視力が零となった。39歳以下でぶどう膜炎を発症した症例，ぶどう膜炎発症から3年以上経過して白内障手術を行った症例，水晶体嚢内摘出術の際にα-キモトリプシンを使用した症例に視力喪失例が多かった。現在，我々はベーチェット病の併発白内障に対する嚢内摘出術の際にはα-キモトリプシンを使用していない。

We evaluated the outcome of surgery in 29 eyes in 18 patients with Behçet's disease during a 16 -year period. All the cases were followed up for one year or more. The series comprised 16 males, 26 eyes, and 2 females, 3 eyes. We performed intracap-sular cataract extraction in all the eyes but one.Final visual acuity of 20/40 or better resulted in 7 eyes. Another 7 eyes became blind. Blindness was often associated when the uveitis developed at the age of 39 years or earlier, cataract surgery was performed after presistence of uveitis for 3 years or longer, and when alpha-chymotrypsin was used during surgery.
