硝子体手術を施行したぶどう膜炎症例の白内障手術

Cataract surgery following vitrectomy in uveitis eyes

　網膜剥離，硝子体混濁，硝子体出血に対して硝子体手術を施行したぶどう膜炎27眼中の17眼（63％）に白内障手術を追加した。11眼には白内障手術を単独で追加し，その中の2眼には眼内レンズ（後房レンズ）挿入術を施行した。6眼には硝子体手術を追加する際に白内障手術を同時に行った。硝子体手術から白内障手術までの期間は0.5か月から1年9か月（平均8.4か月）であった。最終視力が0.1以上であった症例は白内障手術単独追加例では10眼（90.9％）であったが，硝子体手術と同時追加例では1眼（16.7％）であった。白内障手術を単独で追加する場合に眼内レンズ挿入術を行うかどうかは，適応を慎重に検討する必要がある。

We performed vitrectomy in 27 eyes with various types of uveitis manifesting retinal detachment, vitreous opacity or hemorrhage. Cataract surgery was performed in 17 of the 27 eyes, 63%, includingsimple cataract extraction in 11 eyes and additional intraocular lens implantation in 2. In 6 eyes, catar-actous lens was removed during additional pars plana vitrectomy. The interval between the last vitrectomy and cataract surgery ranged from 0.5 to 19 months with a median of 8.4 months. Final visual acuity of 20/200 or better was achieved in 10 of 11 eyes with simple cataract extraction and in 1 of 6 eyes with combined lens removal.
