中心性漿液性網脈絡膜症のパターンERGによる検討

Pattern electroretinogram in central serous cho-roidopathy

　中心性漿液性網脈絡膜症（CSC）患者29例においてパターンERG （PERG）を記録し，正常対照群18人の結果と比較検討した。PERGの評価には陽性波のP1とそれに続く陰性波N2の振幅と潜時を用いた．CSC患者のうち黄斑部網膜剥離のあるもの17例中9例（52％）でP1潜時が正常域より延長し，15例（88％）でN2振幅が正常域より減少していた。これに対して黄斑部網膜剥離の復位していたもの12例中，P1潜時が延長していたのは2例（17％），N2振幅が減少していたのは4例（33％）であった。視力，発症からの期間および中心暗点の有無について，PERGの各成分とを比較検討したが，相関はなかった。PERGはその刺激が後極部に限局し，神経節細胞からの応答が期待でき，視力，中心暗点などの黄斑部機能の自覚的指標では表し得ない黄斑部異常を客観的に評価できると考えられ，CSCの経過を観察するうえで有用な他覚的指標と考えられる。

We recorded the pattern electroretinogram (PERG) in 29 eyes with central serous chor-oidopathy. We paid particular attention to the amplitude and latency of P1 and N2 components. P1 and N2 components in the affected eyes showed prolonged latencies and reduced amplitudes when compared with the healthy control eyes. These features were more pronounced regarding thelatency of P1 and amplitude of N2. These changes recovered to some degree after reattachment of the macula but were still detectable in some. The PERG finding was not correlated with the visual acuity, duration of the disease or the severity of central scotoma. PERG appeared to be useful in estimating the degree of recovery of retinal func-tion.
