棒状異物による下外側結膜嚢打撲から発症した眼窩先端症候群の1例

A case of orbital apex syndrome following trauma to the infratemporal conjunctival sac by a stick

（17-P2-33）　症例は8歳女児で，前かがみになつたとき，玄関の植木の支柱で右眼を受傷し，右眼の視力低下と眼瞼下垂と眼球運動障害が生じた。プレドニゾロンの投与により回復した。受傷状況やCTのガス像から，下外側球結膜から眼窩内に支柱の先端が刺入し，炎症が深部に波及し眼窩先端症候群を発症したものと考察した。

An eight-year-old girl was hit by a prop of flowerpot in her right eye when bending forward. She developed decreased vision, blepharoptosis and impaired ocular motility. She showed laceration in the infratemporal conjunctival sac in the affected eye. Computed tomography show an air bubble along the inferior floor of the orbit. The symptoms disappeared following systemic prednisolone. The orbital apex syndrome was interpreted as consequence of inflammatory expansion of the orbital apex by the penetrating stick.
