We reviewed 28 cases of bilateral occipital infarction regarding the course of visual acuity and visual field as related to the underlying systemic features. We could classify 20 of the 28 cases into three groups based on Goldmann perimetric findings : those with preserved central field (12 cases) , those with intact peripheral field with central scotoma (5 cases) and those with severe field loss leaving peripheral island only (3 cases) . The first two groups resulted in fair visual acuity regardless of the status of visual acuity at the initial examination. The third group resulted in unfavorable visual outcome. Fair visual outcome was the rule in 7 cases with cardiac lesions including arrhythmia and ischemic heart diseases.
