流涙と結膜充血で発症した眼窩骨腫の1例

Epiphora and conjunctival injection preceded manifestation of orbital osteoma

　右眼の流涙と結膜充血で発症し，進行性の眼球突出を主訴とした，25歳女性の眼窩骨腫の1症例を報告した。眼窩骨腫は比較的稀な腫瘍であるが，その主症状はほとんどが眼球突出や眼球運動障害である。本症例も眼球突出を主訴としたが，その前駆症状として，流涙や結膜充血を呈した珍しい症例であった。眼窩骨腫のほとんどは篩骨洞から発生するものであり，その部位や人きさによっては，流涙が発生する可能性がある。本症例では，眼球突出が発症するまでの約5か月間，患者は流涙や結膜充血を自覚していたが放置していた。眼窩骨腫は発育の遅い腫瘍であるが，大きくなると視神経障害や眼球運動障害などが生ずることもあり，本症例のような流涙や結膜充血は，早期の徴候のひとつとして重要と思われた。

A 25-year-old female was referred to us with a 9 -month history of epiphora and conjunctival injec-tion in her right eye. She also noted progressive exophthalmos since 4 months before. Her visualacuity was 20/20 in either eye. The right eye was proptotic by 5 mm. Slight constriction in the tempo-ral visual field was present in the right eye. Plain skull x-ray and computed tomography showed a calcified mass in the right ethmoidal sinus extend-ing into the right orbit. The excised mass was very solid and measured 32×36×24 mm. Histopathology showed it to be an orbital osteoma with dense bone formation. This case seemed to illustrate the impor-tance of epiphora and conjunctival injection as early clinical signs of orbital osteoma originating from the ethmoidal sinus.
