It has been known that the pupil in the affected side by cavernous sinus lesions is usually normal or miotic in spite of oculomotor nerve involvement. This phenomenon has been attributed to a coinci-dental involvement of ocular sympathetic fibers surrounding the internal carotid (Meadows 1959).

In order to verify this hypothesis, we evaluated 25 patients with cavernous sinus syndrome by means of quantitated ocular movement tests and pharmacological pupillary reactions.
