上部角膜上皮弓状病変を呈したドライアイの症例

A case of dry eye with bilateral superior epithelial arcuate lesion

（18-P2-27）　上部角膜上皮弓状病変を呈した症例を報告した。症例は18歳の女性で，初めてのソフトコンタクトレンズを約15か月の間，蛋白除去をせずに1日約16時間装用し，定期検査も受けていなかった。両眼眼痛を主訴に当院を受診し，上記の病変を認めた。綿糸法にて涙液量を測定したところ8mmと低値で，「ドライアイの疑い」例に相当した。非含水性レンズを処方し，人工涙液の併用にて良好な経過を得ている。本病変はコンタクトレンズと角膜との磯械的摩擦により生じると考えられている。本症例ではドライアイであることが，本病変の発症に影響を与えたと考えた。

A 18-year-old female had started wearing soft contact lenses 15 months before. She had been wearing them for about 16 hours a day and taken no adequate care no examination by an ophthalmologist. She sought medical advice for bilateral ocular pain. She showed bilateral superior epithelial arcuate lesions (SEALs) . Phenol red thread test showed signs of dry eye. The SEALs in this case appeared to be due to mechanical irritation by the contact lens and decreased tear secretion.
