両側性眼窩嚢腫を伴う視神経コロボーマと両側性脈絡膜コロボーマを呈した兄弟例

Bilateral optic nerve colobomas with orbital cysts and choroidal colobomas in heterozygotic twins

　生後2か月の男児。眼球運動の異常を主訴に受診し，画像診断と眼底所見から両側性眼窩嚢腫を伴う視神経コロボーマと診断された。眼球はやや小さく，眼底には両眼視神経コロボーマと右眼網膜剥離とを認めた。MRIで嚢腫は両側眼窩後部の筋円錐内にあり，その境界は明瞭で内部は分葉状を呈し，内腔が硝子体とほぼ等信号の腫瘤陰影がみられた。この嚢腫は視神経入口部付近で眼球と密着していた。この患児の二卵性双胎の第一子の眼底には両側性脈絡膜コロボーマがみられた。以上からこれらの異常は，胎生裂の閉鎖不全によるもので家族性の要因も疑われた。

A 2-month-old male infant was referred to us because of abnormal ocular motility. He wasdiagnosed as bilateral optic nerve colobomas with orbital cysts. Retinal detachment was also present in theright eye. Magnetic resonance imaging showed bilateral orbital cysts attached to the eyeballs. The cystsshowed clear margins with lobulated walls. The signal intensity was similar to that of the vitreous. Hisheterozygotic twin sister showed bilateral small choroidal colobomas. These abnormalities were interpretedas the result of imperfect closure of embryonic fissure of possible hereditary nature.
