腺様嚢胞癌（adenoid cystic carcinoma）は頭頸部に生じる悪性腫瘍の1つである。涙腺，耳下腺，顎下腺などのほか，口腔内や鼻咽頭，副鼻腔などの小唾液腺からも発生し^1）2），生じる部位により臨床症状，画像所見は多彩である。今回我々は両側眼窩尖部から両側海綿静脈洞を中心に病変を認めるものの，明らかな原発腫瘤が認められなかったことから術前診断に難渋した腺様嚢胞癌の1例を報告する。

We report a rare presentation of adenoid cystic carcinoma of a 70-year-pld female. Radiological examinations showed infiltrative masses mainly located in the both orbital apexes and the bilateral cavernous sinuses. Symmetrical distribution of the lesions and the characteristic sclerotic bony invasion around the sphenoid sinus may indicate the adenoid cystic carcinoma of sphenoid sinus origin.
