　大量の網膜色素上皮下出血に続発したと考えられる大型の網膜色素上皮裂孔の2例を経験した。併発した網膜下出血に対し，血腫除去術を行い良好な視力を得た。2例とも，併発した大量の網膜下出血のため術前に色素上皮裂孔を確定することは困難であったが，検眼鏡的所見から色素上皮破孔を疑った。発症機序としては，通常の色素上皮剥離に伴うものではなく，脈絡膜新生血管からの出血が色素上皮下で増加し網膜色素上皮に亀裂を生じたと推測された。大量の網膜下出血では，色素上皮裂孔の存在を疑い，特に血腫除去術を行う際には剥離している網膜色素上皮を誤吸引しないよう注意すべきであると思われた。

We observed two cases, aged 46 and 57 years, who manifested giant tear of the retinal pigmentepithelium (RPE) secondary to massive hemorrhage in the sub-RPE space.Surgical removal of hemorrhagicmass resulted in good visual acuity. Recognition of tear of the RPE was difficult befor surgery. It appearedthat accumulation of hemorrhagic mass in the sub-RPE space induced the giant tears.
