　1歳1か月の男児。左眼外斜視および白色瞳孔の精査目的で来院した。左眼底の後極部に視神経を中心とする広範囲の非定型的脈絡膜コロボーマがみられた。催眠下閃光刺激視覚誘発電位（FVEP）および網膜電位図（ERG）の同時記録においては，FVEPで左眼の主要成分の低振幅かつ潜時の延長がみられ，ERGでは左眼の反応は低振幅で基本波形がみられなかった。眼窩部CTでは左眼軸長は27mmと延長し，かつ脈絡膜コロボーマに一致した後極部の著しい菲薄像をみた。ヘリカルCTを用いた3D-CTではendoscopy像とvolume rendering像で欠損部の菲薄像が描出されたが，各組織のCT値の差が少ないため詳細な識別は困難であった。

A 13-month-old male infant presented with leukocoria and exotropia in his left eye. The affected eye showed an atypical choroidal coloboma involving the peripapillary area. Flash visual evoked potential (FVEP) under hypnosis showed a small amplitude with prolonged latent time in the affected eye. Simultaneously performed electroretinogram (ERG) showed a poor response. Computed tomography (CT) showed 27mm as the axial length of the left eyeball and a thinning in the posterior fundus area corresponding to the site of choroidal coloboma. Three-dimensional helical CT also showed a thinning in the posterior fundus area both by endoscopic and volume-rendering imaging. More detailed imaging was not possible bue to low threshold of the lesion.
