脈絡膜コロボーマに網膜分離症様変化を合併した黄斑網膜剥離の治療経験

A surgically treated case of choroidal coloboma with retinoschisis and macular detachment

（P1-1-8）　脈絡膜コロボーマに網膜分離症様の稀な形を合併した黄斑網膜剥離の治療経過について報告した。症例は22歳，男性。両眼脈絡膜コロボーマの診断を受けていたが，1か月前からの変視症を主訴に受診した。初診時の左眼矯正視力は0.7で，脈絡膜コロボーマに接する円形の網膜分離症様の漿液性剥離と，その内部に外層網膜剥離が存在した。硝子体の液化は著明であった。視力低下が進行し，硝子体手術，意図的後部硝子体剥離作成，ガスタンポナーデを施行した。術後，視力は1.2に回復し，網膜分離症様所見と黄斑網膜剥離も消失したことにより，硝子体の牽引が病態に関与していると考えられた。

A 22 -year- old male presented with metamorphopsia and impaired vision in his left eye since one month before. He had earlier been diagnosed as bilateral choroidal coloboma. The corrected visual acuity was 0.7 left. Funduscopy showed serous retinal detachment simulating retinoschisis adjacent to the coloboma and involving the macula. The affected eye also showed advanced vitreous liquefaction. We performed vitrectomy, intentional posterior vitreous detachment and gas tamponade. The visual acuity improved to 1.2 four months after surgery. Macular detachment with retinoschisis disappeared also. It appeared that vitreous traction was the major factor for the observed retinal lesions.
