A 68-year-old male was referred to us for cataract surgery. He had developed repeated vitreous hemorrhage as complication of branch retinal vein occlusion in the left eye since 4 years before. When seen by us, the left visual acuity was counting fingers at 20 cm for mature cataract. Ultrasonography showed normal findings in the vitreous cavity. When seen one month later, the posterior capsule had ruptured with loss of lens nucleus. Ultrasonography showed a mobile mass in the vitreous cavity. Pars plana lensectomy and anterior vitrectomy led to the detection of lens nucleus which had dislocated into the vitreous. It appeared that a spontaneous break of the posterior capsule led to dislocation of the lens nucleus.
