We treated 23 eyes with glaucoma by thermal sclerostomy using a thulium, holmium, chromium -doped YAG crystal laser (holmium YAG laser). The success probability to maintain the intraocular pressure below 21 mmHg with or without medication, estimated by life-table method of Kaplan-Meier, was 78% for primary open angle glaucoma and 70% for secondary open angle glaucoma. The postoperative astigmatism disappeared within 6 months. It tended to persist in eyes which received multiple laser shots. The holmium YAG laser sclerostomy appeared to be of value because of lesser conjunctival trauma, no intraocular manipulations and short duration of surgery. Iris carceration into the sclerostomy site was present in 68%. Its prevention appeared to be necessary to achieve better success.
