We performed astigmatic keratotomy in 18 eyes with against-the-rule astigmatism after primary or secondary intraocular lens implantation surgery. We placed two 4 mm transverse relaxing incisions leaving a 6 mm optical zone. The depth of incision was designed to equal the thickness of central cornea.

The amount of corneal astigmatism averaged 3.65D before keratotomy and 0.82D 6 months after surgery. The thickness of central cornea increaed in the immediate postoperative period and returned to the initial value one months after surgery. The re -epithelialization of the incised wound became complete in 2 months. The density of corneal en-dothelial cells remained the same in the central cornea 6 months after surgery. The cell density around the incision decreased by 2.6%.

It is presumed that astigmatic keratotomy is effective for reducing postoperative astigmatism and that no serious complication is incurred to the corneal endothelial cells during the 6-month post-operative period.
