Acute attack of bronchial asthma developed in 4 cases following instillation of timolol ophthal-mic solution. In the first case, prolonged instillation of 0.5% timolol was uneventful 6 years before. Afterdiscontinuation of timolol, bronchial asthma was diagnosed 4 years before. Acute attack of asthma devel-oped immediately after instillation of 0.25% timolol. In the second case, bronchial asthma started to recurafter treatment of glaucoma with timolol for 16 years. In the third case, a single instillation of 0.5% timololwas follewed by an asthmatous attack. In the fourth case, a major attack of bronchial asthma with cardiacarrest developed following timolol instillation for 15 months. In all the 4 cases, withdrawal of timololresulted in absence of further asthmatous attacks. Topical instillation of betaxolol in the first 2 casesinduced no asthmatous attack nor changes in spirometric parameters. Betaxolol appears to be a safesubstitute for timolol in patients with glaucoma and chronic lung disease.
