虹彩浸潤を最初の再発所見とした急性リンパ性白血病の成人例

Iris infiltration as the first sign of relapse in an adult with acute lymphocytic leukemia

　再発所見として最初に虹彩浸潤をきたした成人の急性リンパ性白血病（acute lymphocytic leuke—mia：ALL）を経験した。症例は38歳男性。主訴は左眼霧視で，既往にALLがあり完全寛解が得られていた。両眼とも視力は1.5，眼圧，中間透光体，眼底は正常だったが左眼に虹彩炎様症状と虹彩全体の腫脹がみられた。副腎皮質ステロイド治療を開始したが反応せず，急性緑内障発作を生じたため，治療と生検を兼ね周辺虹彩切除術を行った。切除虹彩の組織像からALLの虹彩局所再発と診断されたが、全身的には異常はなかった。直ちに化学療法を開始したところ，約2週間で虹彩はほぼ正常となった。副腎皮質ステロイドに反応しない虹彩炎の原因として血液疾患も念頭におく必要がある。

A 38-year-old male presented with blurring of vision in his left eye. He had been diagnosed 20months before as acute lymphocytic leukemia, ALL. Complete remission was achieved now by repeatedsessions of chemotherapy. The left eye showed diffusely thickened iris with 2+ cells and flare in the anteriorchamber. Topical corticosteroid was futile and the left eye developed hypopyon and an acute attack ofglaucoma. Iris specimen obtained during peripheral iridectomy showed infiltration of leukemic lymphoblastsin the iris stroma. Systemic chemotherapy induced rapid improvements and return to normal findings. Thiscase illustrates that the iris is a potential site of relapse of ALL even in adults.
