Color Doppler imaging was performed to detect the superior ophthalmic vein (SOV) in 34 eyes with dysthyroid ophthalmopathy. The series included 15 with dysthyroid optic neuropathy. We used an EUB 565A unit (Hitachi) with a 7.5 MHz probe. In 7 orbits, the blood flow in the SOV was reversed, or posteroanteriorly. Computed tomography in these cases showed crowding in orbital apex, indicating enlar-ged extraocular muscles. Dysthyroid optic neuropathy was present in 6 of the 7 orbits with reversed flow and in 9 of 27 orbits without. The difference was significant (p<0.05). By fluorescein angiography, the intra-retinal transit time was longer in 7 eyes with reversed flow than in the remaining 27 eyes (p<0.05). Above findings show the presence of venous stasis in the orbit, which may play a role in the pathogenesis of dysthyroid optic neuropathy.
