　ゴールドマン圧平眼圧計による眼圧測定で21mmHg以上の高眼圧が検出された40例66眼の甲状腺眼症に対してCTスキャンによる眼窩水平断，冠状断の撮影を行った。対象66眼中60眼（91％）では少なくとも1筋以上の筋肥大を認めた。対象の眼圧（平均24.0±3.0mmHg）は筋肥大のない25例25眼の平均眼圧（17.1±2.4mmHg）に比べ明らかに高値を示した（p＜0.01）。筋肥大を認めた60眼では下直筋，上直筋のいずれかまたは両者の肥大があった。正面位での眼圧（平均23.4±4.6mmHg）が下方視（平均19.1±3.1mmHg）に比べ明らかに高値を示した（p＜0.01）ことから，外眼筋肥大がもたらす垂直方向の眼球運動障害と高眼圧に関連があることが推定された。

We performed computed tomography (CT) of the head in 40 cases of dysthyroid ophthalmopathy. Elevated intraocular pressure (IOP) above 21 mmHg by Goldmann applanation tonometer was present in 66 eyes of the 40 patients. Axial or coronal CT sections showed enlargement of one or more extraocular muscle, invariably involving the inferior and/or superior rectus, in 60 eyes, 91%. The IOP averaged 23.4±4.6 mmHg in the primary posi-tion and 19.1±3.1 mmHg in the downward gaze which was the neutral position in the patients. The difference was significant (p＜0.01). The findings show that the observed ocular hypertension was mainly due to disturbed vertical eye movement secondary to enlargement of extraocular muscles in dysthyroid ophthalmopathy.
