Abstract 文献概要

　緑内障や阻血性視神経症^＊＊において，中心視野の障害が比較的早期に先行するものがあり，こうした症例では耳側コーヌスを認める例が多いとの臨床的印象があった．今回はこの印象を証明する目的で，コーヌスと中心視機能との関連を検討した．

　対象　は原発開放隅角緑内障（POAG）241眼，発育緑内障（DG）38眼，低眼圧緑内障（LTG）48眼，阻血性視神経症（ION）63眼，原発閉塞隅角緑内障（PACG）76眼を含む，計431名，861眼である．

　螢光眼底写真の30秒前後でよく撮れたものを選び，耳側における半月状（輪状）の背景螢光のブロックの幅を乳頭の縦径と比較して，T1（〜1／4DD），T2（1／4〜1／2DD），T3（1／2DD〜）のように判定した．Tは耳側コーヌス，Aは輪状コーヌスを示す．

　0.6以下の視力の頻度はT1,T2,T3の順で13％，19％，34％と増加した．A1,A2,A3の場合は20％，47％，58％と大きな差が認められた（650眼での検討）．

　オクトパスNo31によって計測された結果をデルタ解析し，0〜10°の平均dB値を求めた．この値が20dB以下の例の頻度はT1,T2,T3で，21％，33％，57％と増加し，A1,A2では33％，61％と増加した．

　以上により冒頭に述べた臨床的印象が証明された．

We evaluated the correlation among the size of conus, visual acuity and the central visual field in a total of 861 eyes. The series included primary open angle glaucoma 241 eyes, developmental glaucoma 38 eyes, low tension glaucoma 48 eyes, ischemic optic neuropathy 63 eyes and primary angle closure glaucoma 76 eyes. The size of the conus was graded according to the fluorescein angiogram in each eye. When the width of the conus was less than 1/4DD (disc diameter), the eye was graded as T1. It was graded as T2 or T3 when the width was less ormore than 1/2DD respectively. Tokyo The incidence of impaired visual acuity of less than 0.6 was 13% for T1, 19% for T2, and 34% for T3.

We then evaluated the central vision by autoper-imeter Octopus 201, program No 31. We calculated the average dB for the central visual field within 10ｰ from the fixating point. The incidence of impair-ed central vision not exceeding 20 dB was 21% for T1, 33% for T2 and 57% for T3. The findings show that the presence of temporal conus in glaucoma and ischemic optic neuropathy suggests the possibility of an early involvement of central visual field and vision.
