We performed computed tomography (CT) of the head in 40 cases of dysthyroid ophthalmopathy. Elevated intraocular pressure (IOP) above 21 mmHg by Goldmann applanation tonometer was present in 66 eyes of the 40 patients. Axial or coronal CT sections showed enlargement of one or more extraocular muscle, invariably involving the inferior and/or superior rectus, in 60 eyes, 91%. The IOP averaged 23.4±4.6 mmHg in the primary posi-tion and 19.1±3.1 mmHg in the downward gaze which was the neutral position in the patients. The difference was significant (p<0.01). The findings show that the observed ocular hypertension was mainly due to disturbed vertical eye movement secondary to enlargement of extraocular muscles in dysthyroid ophthalmopathy.
