結膜悪性黒色腫の治療

Treatment of malignant melanoma of conjunctiva

　症例は66歳，女性。主訴は右眼眼瞼結膜の腫瘤および色素沈着。幼少時より右眼球結膜にあった褐色の色素沈着の範囲が広がって，長崎大学眼科を受診した。結膜悪性黒色腫の疑いで経過観察中に脳梗塞を起こし，以後受診しなかった。5年後の再診時には，右眼上眼瞼結膜耳側の眼瞼縁近傍に黒褐色で有茎の腫瘤があり，上下眼瞼結膜と球結膜に広範な色素斑があった。腫瘤とその周囲の眼瞼のみを切除し，眼瞼機能と視機能を温存した。病理組織学的には，色素顆粒を伴った紡錘型ならびに類上皮型細胞による悪性黒色腫であった。術後6か月，再発転移はない。長期間にわたる眼瞼・球結膜メラノーシスが悪性化した場合，その進行が緩徐であり，病巣が広汎であることから，眼窩内容除去術ではなく局所的眼瞼切除術でもよいと考えた。

A 66-year-old female presented with diffuse pigmented lesion in the palpebral and bulbar conjunctiva in the right eye since 50 years before. The lesion started to enlarge 6 years before and become nodular 1 years before. The tumor was partially excised together with full-thickness eyelid. Histological studies showed spindle and epithelioid cells with pigmentation leading to the diagnosis of malignant melanoma. There were no signs of local recurrence or systemic symptoms for 6 months after excision. Regarding the extent of the tumor and its slow growth, conservative local excision appeared to be indicated instead of orbital exenteration.
