結膜悪性黒色腫の1例

A case of conjunctival malignant melanoma

　56歳男性が6か月前からの左眼の腫瘤，異物感，眼痛で受診した。幼児期より左眼の球結膜に色素斑があったが，最近まで変化はなかった。左眼には，外下方の輪部に接する球結膜に茶褐色の結節性腫瘤，扁平な色素斑，色素沈着があった。これ以外には色素異常はなかった。切除した腫瘤は悪性黒色腫であることが病理組織学的に確定した。さらに広範囲球結膜切除術，冷凍凝固術，局所化学療法を行ったが，6か月後に局所再発した。再手術を行い，以後8か月間の経過は良好である。

　A 56-year-old male presented with a tumor,foreign body sensation,and ocular pain in his left eye since 6months before. A pigmented patch had been present in his bulbar conjunctiva since childhood. His left eye showed a pigmented nodular tumor and pigmented patches in the bulbar conjunctiva lower-temporal to the limbus. The resected tumor was diagnosed with malignant melanoma. He then received radical resection of bulbar conjunctiva,cryocoagulation,and application of mitomycin C. The melanoma recurred another 6months later and was treated by similar procedures. He has been doing well during the ensuing 8months.
