結膜悪性黒色腫の2例

Two cases of conjunctival malignant melanoma

　結膜に発症した悪性黒色腫2例を経験した。症例1は80歳の女性で，20歳頃に自覚した母斑から発症したもので，角膜輪部結膜から急速に増大する結節性の腫瘤と眼瞼縁まで拡がる黒褐色の色素斑を認めた。症例2は66歳の男性で，角膜輪部結膜に結節性の腫瘤と瞼結膜から眼瞼縁にかけて拡がる黒褐色の色素斑があり，角膜に浸潤していた。両症例とも腫瘍は球結膜に限局せず広範囲に拡がり，症例1ではリンパ管内への浸潤があり，予後不良と考えられた。両症例に対して眼窩内容除去術を行い，転移の危険を軽減するために免疫・化学療法を併用した。以降それぞれ14か月と8か月の観察期間中，経過は良好である。

We observed malignant melanoma of the bulbar conjunctiva in a female and a male aged 80 and 66 years each. Both cases manifested a rapidly enlarging nodular tumor from the corneal limbus with pigmented patches extending to the lid margin. In the first case, it apparently originated from a conjunctival pigmented nevus present since 20 years of age. Because of widespread invasion suggestive of poor prognosis, both cases underwent orbital exenteration followed by systemic chemotherapy. There has been no local recurrence or metastasis after the treatment.
